Investigations into incidents involving ‘human error’ rarely attempt to understand causal factors and why the human error occurred. Identifying both the immediate and underlying causes of the incident is key to preventing similar incidents from occurring in future through the use of effective control measures.

Ergonomie is experienced in conducting and managing incident investigations in high-risk, complex environments. This is facilitated by our knowledge and understanding of human behaviour, cognition and physical attributes, as well as job, individual and organisational factors which may directly affect human behaviour.

 
 

Our experience


Ergonomie has expertise in the identification of underlying causal factors in incidents and the development of tailored and targeted recommendations to address the issues and reduce the likelihood of a similar incident occurring. Specific examples include:

  • Reviewing historic incident data to support consistent coding of incidents (using the Contributing Factors Framework (CFF)) to improve understanding of trends and identification of targeted management strategies

  • Providing Human Factors subject matter expert input to incident investigations

  • Provision of mentoring and coaching to incident investigators to ensure systemic issues are identified and addressed within complex organisations

  • Development and delivery of human factors training for incident investigators
